ParseDiagnostic	diagnostic	Failure that marked the span; surfaced verbatim while pending. Its range tracks the span across later edits.
size_t	end = 0	End byte offset in current source coordinates (exclusive).
uint64_t	sequence = 0	Recency order for surfacing; higher is a newer failure.
size_t	start = 0	Start byte offset in current source coordinates (inclusive).
